Forms & Leads

Forms & Leads turns the control panel into a form builder. Build as many forms as the site needs — every submission is stored in your own database and, at the same moment, lands in your inbox and in Telegram.

• Twelve field types: text, textarea, e-mail, phone, number, date, select, radio buttons, checkboxes, single checkbox, consent and hidden. Each field is half- or full-width, required or optional.
• Any number of forms, each with its own slug — embed one into a template or open it as a standalone page at /form/{slug}.
• Submissions are shown as a table whose columns are built from the form itself; statuses «new» and «done», per-form counters and CSV export.
• Notifications: a list of e-mail recipients for every form plus an optional Telegram message through your own bot.
• Honeypot anti-spam, CSRF-protected endpoint, and a value / conversion label pair for Google Ads goal tracking.

Nothing to wire up by hand: install the plugin, and the «Leads» section with the default form is already in the admin panel.
